As part of the GMS contract we have a contractual requirement to notify our patients that they each have a named GP who is responsible for their overall care at the practice.
In the last year we have been notifying patients opportunistically who their named GP is. If you haven’t been notified, please contact the practice where a member of reception will be able to answer your query.
Although all patients have a named accountable GP, this does not stop you from seeing a different GP should you wish to.

Advanced Clinical Practitioners
Advanced Clinical Practitioners (ACPs) are educated to a Master’s level in advanced practice and assessed as competent at this level. As clinical leaders, they have the autonomy and authority to make decisions, accepting full responsibility and accountability for their actions. Their role is defined by a high level of independent decision-making, encompassing patient assessment, diagnosis, treatment—including prescribing—for individuals with complex, multi-faceted health needs.

General Practice Nurses
General practice nurses are highly skilled members of the primary care team who provide direct patient care to our practice population. Each nurse is responsible and accountable for their own actions and work safely within a Scope of Professional Practice.
The are all independant prescribers, who offer vast amounts of screening, health promotion and education, whilst playing a crucial role in managing long term conditions: cardiovascular disease, hypertension, respiratory disease, diabetes, sever mental illness, learning disabilities.
They offer an extensive womens health service which includes assessment, treatment and management of menopause, family planning, continence, contraception and STI checks,
The practice is proud to offer a nurse led vaccination service, which includes all aspects of immunisations, including childhood and travel.

Registered Nurse Associate
A nursing associate in primary care is a hands-on, person-centred professional who works collaboratively with registered nurses and healthcare support staff to provide comprehensive care. They play a crucial role in delivering high-quality, patient-focused services across a range of areas. Their responsibilities include carrying out chronic disease management, such as reviewing and monitoring conditions like hypertension, diabetes, and cardiovascular disease. They are also involved in conducting health checks for individuals with severe mental illness (SMI), supporting people with learning disabilities, and delivering immunisation and screening programmes. By working closely with patients, they help to ensure that individuals receive the appropriate care and support tailored to their needs, contributing to improved health outcomes in the primary care setting.

Healthcare Assistant
Supports the nursing team by providing an essential serviceto patients, carrying out tasks such as venepuncture, ECGs, Health checks, blood pressure monitoring, foot checks, weight monitoring and some aspect of immunisation.
Reception / Admin
We have a team of full and part-time receptionists/admin staff who are here to help you. They will ask you for further details when you request to book an appointment. This is so they can assist you as quickly as possible. You can be reassured that they will treat your request sympathetically and will respect the rules of confidentiality in the same way as doctors and nurses.
Medical Secretaries
We have two secretaries, Marie and Adele, who both deal with referrals, requests from solicitor and forms for completion.
Practice Support Officer / Practice Digital Lead
Lisa can help guide you regarding Patient Access, NHS App and Proxy applications. She is also responsible for updating the practice website to help keep you informed of any changes or useful information services.
Treatment Room Nurse
We have a Treatment Room Nurse who holds a Treatment / Dressing Clinic at the surgery on Monday – Friday 08.45 – 12.15. Booking for this clinic should be made on the Central Booking telephone number 0161 763 8525 between 09:00 – 12:30 and 13.30 – 16.00.
Community Nurses
The District Nurses are no longer based at Castleton Health Centre, however they can still be contacted and accessed via the Liason department on 01706 676 363.
Health Visitors
A health visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social wellbeing.
